Wolverine Athletics Employs New PRIVIT Document System
A Letter from Our Atletic Director Regarding Atheltic Forms:
Wolverine Family:
I hope all of you are having a wonderful and relaxing summer break! As the new school year approaches, I wanted to communicate some updates and changes in what is required and how PACS will store and update the medical records needed for your child to participate in athletics. Student safety remains our number one priority.
In the last legislative session, the state of Georgia passed SB 60, the Jeremy Nelson and Nick Blakely Sudden Cardiac Arrest Prevention Act. A major piece of this law requires that all schools clearly communicate the symptoms and warning signs of sudden cardiac arrest. Each 6th-12th-grade student participating on any level of athletic team at PACS will now be required to have three forms completed before beginning practice or competition. These forms are the GHSA Physical form, the Concussion Awareness form, and the Sudden Cardiac Awareness form. Every 6th-12th-grade student will be required to have all three of these forms completed before they begin participating. This includes 6th graders competing in non-middle school activities such as 5th and 6th-grade football and 6th-grade basketball.
PACS will be persistent in making sure every participating student turns in and maintains these required forms. In an effort to better serve and protect our students, PACS has begun a relationship with PRIVIT.
PRIVIT is a streamlined, electronic tool that we will be using to collect, store, and manage your student’s health information. It will allow our coaches and trainer to increase the safety of our athletes and enhance the accessibility and security of the health information when needed while also being HIPPA and COPPA compliant.
Each parent of an athlete is asked to do the following:
- Please read and follow the directions in the PRIVIT welcome letter. Click Here for Letter. (We have found that it might be easier for you to print out the directions to follow them.) You will be given a link to our PRIVIT site to register as a parent or guardian.
- You will then need to add each child as a “member”. You will be required to fill out the first page of the Pre-Participation Physical form regardless of whether you have already turned in a 2019-20 form to the office. Note-once you have completed this page of the form, it will be automatically stored and updated for future years at PACS.
- If you have not turned in a Physical form for 2019-20, you will be asked to take a picture of your completed form and upload it directly to PRIVIT. If you have already turned in the 2019-20 Physical form, PACS will upload the form once you have completed Steps 1 and 2.
- Please read and digitally sign the Concussion Awareness and Sudden Cardiac awareness forms.
I know this is a big change, but I believe it will be a great benefit to you and PACS as we strive to protect and serve our students. I am confident you will find the process easier once you learn how to use it.
